Small Business Fund Application

Neighborhood Brick-and-Mortar Small Business Fund

This application is used to collect information necessary to determine applicant eligibility and to evaluate proposed projects for grant funding. Applicants should review all program guidelines carefully before applying. 


Submission of an application does not guarantee funding. Applications that meet eligibility requirements will be reviewed and evaluated based on established program criteria. 


Applicants are encouraged to review the Required Documents section in advance and ensure that all required materials are available before submission. Incomplete applications may not be considered.

Required Documents 

Applicants must be able to submit all required documentation as outlined below. Applicants selected for a conditional award must submit all post-award documentation within thirty (30) calendar days of notification


This grant is intended to support activities aligned with the funder’s mission and values and does not fund businesses operating in industries such as alcohol, tobacco, gambling, or adult entertainment. 


Applicants must submit the following documents as part of their application:

  • Copy of business permit 

  • Copy of formation documents or organizational paperwork (DBA, LLC, Partnership Agreement, etc.) 

  • If leasing, current lease documentation 


Applicants selected for a conditional award must submit the following documents within thirty (30) calendar days of notification and prior to the release of grant funds: 

  • Current Workers’ Compensation Certificate or approved NY State Workers’ Compensation and/or Disability and Paid Family Leave Benefit Insurance Coverage Waiver (Form CE-200) 

  • Current Disability Insurance Certification or approved NY State Workers’ Compensation and/or Disability and Paid Family Leave Benefit Insurance Coverage Waiver (Form CE-200) 

  • General Liability Insurance certificate of at least $1,000,000 

  • Proof that rent, lease, or mortgage payments are current 

  • If leasing, written landlord approval for any permanent or structural improvements 

  • Before photographs documenting existing conditions of the areas to be improved, to be submitted prior to the start of funded work 


Failure to submit required documentation within the specified timeframe may result in forfeiture of the award. Extensions may be granted at the discretion of the administering organization.
